Front Desk Home Health is a service of Patrick Moon Physical Therapy P.C.
Patrick Moon Physical Therapy P.C. ("PMPT", "we", "us" or "our") provides home health scheduling and care coordination services and operates Front Desk, our scheduling system at frontdeskhomehealth.com (the "Service"). This Privacy Policy explains what information we collect, how we use, share and protect it, and the choices you have, including for text messages.
1. Information we collect
Information about patients. To schedule and coordinate home health visits, we receive information from the home health agencies that refer patients to us, and from patients, their families and caregivers. This includes name, address, phone numbers, date of birth, visit dates and times, the clinicians involved, and other information needed to coordinate care.
Mobile phone numbers and text message consent. If a patient or their representative agrees to receive text messages from us, we keep the mobile number, when and how consent was given, and the messages sent and received.
Calls and messages. We keep records of calls and text messages with our office, including the date, time and length of calls and the content of text messages.
Staff and clinician accounts. For people who use Front Desk, we collect name, email address, role, and a record of activity in the system.
Information collected automatically. When someone uses the Service, our servers record technical information such as IP address, browser type, and the date and time of access, to keep the Service secure and working. We use only the cookies needed to keep users signed in and to secure their session. We do not use advertising or tracking cookies.
2. How we use information
We use information only to provide our services, including to:
- schedule, confirm and reschedule home health visits;
- contact patients and their representatives about their care and visits;
- coordinate with clinicians and the referring agencies involved in a patient's care;
- keep records required by law and by our agreements;
- operate, maintain, secure and improve the Service.
We do not use information for advertising, and we do not sell it.
3. Health information and HIPAA
Information about a patient's health and care is protected health information under the Health Insurance Portability and Accountability Act (HIPAA). We handle it as HIPAA requires and as set out in our agreements with the home health agencies we work with. Access is limited to staff and clinicians who need it for a patient's care, every access to a patient's record is logged, and the service providers that handle it on our behalf are bound by business associate agreements. Patients' rights over their health records are described in the Notice of Privacy Practices of the agency providing their care.
4. Text messaging
If you agree to receive text messages from us, we use your mobile number to send messages about your home health visits, such as confirmations, reminders and schedule changes. You can opt out at any time by replying STOP. Our SMS Terms are at frontdeskhomehealth.com/terms.
We do not sell, rent, or share mobile phone numbers, or text messaging opt-in data and consent, with any third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties, except with the service providers that deliver our messages on our behalf, who may use it only to provide that service.
5. How we share information
We share information only:
- with the clinicians and referring agencies involved in a patient's care;
- with service providers that help us run our services, such as secure cloud hosting and our phone and messaging provider, under written agreements that require them to protect it and use it only for us;
- when required by law, court order or a government authority, or to protect the safety of a patient or others.
We do not sell personal information.
6. How we protect information
Information is stored on secured servers in the United States, encrypted in transit and backed up regularly. Access is limited by role, every access to a patient record is logged, and users are signed out automatically after a period of inactivity. No method of storage or transmission is completely secure, but we take reasonable administrative, technical and physical measures to protect the information we hold.
7. How long we keep information
We keep information for as long as needed to provide our services and to meet our legal and contractual record-keeping obligations. After that we delete it or remove the details that identify a person.
8. Your choices and rights
- Text messages: reply STOP to any message to stop receiving texts, or HELP for help.
- Your information: you may ask what information we hold about you, or ask us to correct it, by contacting us below. Some requests about medical records may need to go to the home health agency providing your care, and we will tell you if so.
- Do Not Track: the Service does not respond to "Do Not Track" browser signals, because we do not track users across other websites.
9. Children
The Service is not directed to children. Information about a minor patient is provided by a parent, guardian or the referring agency, and is handled as described in this policy.
10. Changes to this policy
We may update this policy from time to time. The current version is always available at frontdeskhomehealth.com/privacy with its effective date. If we make a material change to how we use or share information, we will update the effective date and, where appropriate, notify affected users.
