SMS Terms and Conditions

Front Desk Home Health is a service of Patrick Moon Physical Therapy P.C.

These terms apply to text messages sent by Patrick Moon Physical Therapy P.C. ("PMPT", "we" or "us") about home health visits (the "PMPT visit texts" program).

1. What we text

We send messages about your home health care visits: scheduling, visit confirmations, reminders, and changes to your schedule. For example:

"Hi, this is PMPT about your next home health visit. Reply YES to confirm it, or call us to change it. Reply STOP to stop texts."

We do not send marketing or promotional messages.

2. How you agree to receive texts

When we schedule your care, our scheduling staff ask you, or the person who manages your care, whether we may text you about your visits, for example: "Is it OK if we text you about your visits? You can reply STOP anytime." We record your answer. Agreeing to receive texts is not a condition of receiving care, and you can still reach us by phone.

3. How often we text

Message frequency varies with your schedule, typically a few messages per visit.

4. Cost

Message and data rates may apply, depending on your mobile plan. We do not charge for our messages.

5. How to stop texts or get help

  • Reply STOP to stop receiving texts. We will send one message confirming you have been unsubscribed and will not text you again unless you reply START.
  • Reply HELP for help, or call us at (510) 883-8155.

6. Do not text emergencies or detailed health information

Our texts are for scheduling only. In a medical emergency, call 911. Please do not send detailed medical information by text, because text messages are not a secure way to share it. We do not give medical advice by text; call us or your care team instead.

7. Delivery

Wireless carriers are not liable for delayed or undelivered messages. Delivery depends on your carrier and coverage.

8. Privacy

How we handle your information, including your mobile number, is described in our Privacy Policy at frontdeskhomehealth.com/privacy. We do not share your mobile number or text messaging consent with third parties for marketing.

9. Changes to these terms

We may update these terms from time to time. The current version is always available at frontdeskhomehealth.com/terms with its effective date.
